If you have ordered prescriptions from us before you
are not required to fill out another patient questionnaire
or release form. To order, simply follow these two simple
steps:

Fill Out Prescription Order Form:
Please fill out the prescription order form. Please
note all address/phone number changes and any changes
to your medical condition.
In order to view PDF files, you must have the Adobe
Acrobat Reader installed on your computer system. If
you do not already have a copy of a PDF Reader then
click on the logo to install a FREE copy. Once installed
click on the PDF form link below to read and print.

Fax or Mail Form
Please note that all new orders must be accompanied
with an original prescription before orders can be processed.
All refills can be faxed, e-mailed, or Phoned in.

If you have an existing prescription at another drug
store that you would like transferred please provide
a copy of your prescription label.

Turnaround Time:
Please allow 3 business days for order processing and
10-14 business days for shipping. If you have not received
shipment within 3 weeks please contact us and we will
track your package.
Please Note:
Currently the United States Food and Drug Administration
(FDA) limits the quantity of medication we are able
to send at once to a three-month supply. If your prescription
allows, you can order refills every three months. You
do not have to fill out another patient questionnaire
or release form when placing an order for a refill.
Refills can be ordered by calling us toll-free at 1.800.784.2309
or by email (see How To Order). Once all refills have
been used, you will be required to mail a new prescription
or we can call your doctor for additional refills.
By law, we do not ship controlled substances such as
amphetamines and narcotics.
Under pharmaceutical law, all sales on prescription
drugs are final and non-returnable.
